Management of Fetal Myelomeningocele

Initial Management at Presentation

  • Surgery: When no exclusionary criteria are present, inclusion criteria are met, and the mother/family elects fetal repair, the fetus undergoes intrauterine repair before 26 weeks of gestation.
  • Prenatal observation: If criteria are not met and/or the mother/family defer intrauterine surgery, gestation of the fetus continues until natural delivery.

Adjunctive Therapies

  • None: No adjunctive therapies are required.

Follow-up

  • Obstetric: The mother is evaluated at 2 weeks postoperatively to ensure adequate wound healing. Additional high-risk MFM appointments are attended as needed.
  • Fetal: The fetus is monitored once weekly following fetal repair, or more frequently if clinical concerns arise.
